Crisis Services

You Turn Services provides comprehensive crisis services that work as the buffer between community‐based services and “out of home” or institutional placement. It is an effective and efficient way of resolving mental health crisis and preventing future crisis situations. Our comprehensive crisis services are broken down into a crisis system and individual crisis programs; Community Stabilization and Mobile Crisis Response.

Community Stabilization

Community Stabilization

Community Stabilization services are available 24 hours a day, seven days a week, to individuals experiencing a behavioral health crisis. We provide for short-term assessment, crisis intervention, and care coordination.

 

Primary goals are to deliver short-term or ongoing mental health intervention provided in or outside the home, designed to evaluate, manage, monitor, stabilize, and support rehabilitation and recovery, consistent with the individual’s crisis/safety and individual service plan.

Services Include:
  • Brief therapeutic and skill-building interventions

  • Engagement of natural supports

  • Interventions to integrate natural supports in de-escalation and stabilization of crisis

  • Coordination of follow-up services 

  • Linkages and referrals to appropriate community-based services

Mobile Crisis Response

Mobile Crisis Response

Mobile Crisis Response services are available 24 hours a day, seven days a week, to provide for rapid response, assessment and early intervention to individuals experiencing a behavioral health crisis. Services are deployed in real-time to the location of the individual experiencing a behavioral health crisis.

We provide appropriate care, support and supervision in order to maintain safety for the individual experiencing a crisis and others, while avoiding unnecessary law enforcement involvement, emergency room utilization, and/or avoidable hospitalization.

 

The Purpose of this Service is to:

  • De-escalate the behavioral health crisis and prevent harm to the individual or others

  • Assist in the prevention of an individual’s acute exacerbation of symptoms

  • Develop an immediate plan to maintain safety

  • Coordinate care and linking to treatment services to meet individual's needs

Mental Health Skill-Building

Mental Health Skill-Building Services

Our Mental Health Skill-Building program includes educational training and ample support to enable individuals age 18 years and older with significant psychiatric functional limitations to achieve and maintain community stability and independence. You Turn Services helps clients who have significant impairment regarding managing their behavioral, emotional, and mental health.

 

This form of treatment assist individuals with increasing independence, maintaining safety, and ample amount of training in the least restrictive environment. Our staff works persistently to build rapport and create a working relationship with our clients that ensures progressive improvement.

Outpatient Therapy

We provide Outpatient Therapy  services for clients in a variety of locations; practitioner’s office, mental health clinic, individual’s home, or nursing facility. This program helps clients address psychiatric needs thorough counseling, therapy, and medical treatment.

Clients experience one on one therapy with licensed clinicians utilizing therapy tools and training focused on accessing resources supports to strengthen skills such as:

  • Healthy Social and Interpersonal Skills

  • Appropriate Peer Relations

  • Anger Management

  • Impulse Control and Self-regulation

  • Self-expression

  • Problem-solving Skills

  • Evaluation of mental health needs, assessment & diagnosis

  • Individualized treatment plans tailored to specific identified needs

  • Development & implementation of individual service plans

  • Psychiatry referrals to medical professional

Crisis Services

Who is eligible for services?

Our comprehensive crisis services are available to any individual meeting the below criteria, regardless of diagnosis. Individuals must meet all of the following criteria:

  1. The individual must be experiencing an active behavioral health crisis; and

  2. Urgent intervention is necessary to stabilize or prevent escalation of the individual’s behavioral health crisis; and

  3. The individual or collateral contact reports at least one of the following:

    • suicidal/assaultive/destructive ideas, threats, plans or actions

    • an acute or increasing loss of control over thoughts, behavior and/or affect that could result in harm to self or others

    • functional impairment or escalation in mood/thought/behavior that is disruptive to home, school, or the community or impacting the individual’s ability to function in these settings

    • the symptoms are escalating to the extent that a higher level of care will likely be  required without intervention;

  4. Without urgent intervention, the individual will likely decompensate which will further interfere with their ability to function in at least one of the following life domains: family, living situation, school, social, work, or community.
